Millions Displaced, Thousands Killed: What US-Israel War with Iran Brings to Middle East

While the war between Iran, the United States, and Israel continues, international organizations are reporting a rise in civilian casualties, widespread displacement, and extensive damage to critical infrastructure across several Middle Eastern countries. According to these organizations, millions of people in the region are facing the direct consequences of the conflict, including shortages of essential services, pressure on healthcare systems, and disruptions to daily life.

Children and Humanitarian Impact

In a UNICEF report published on March 11, 2026, it was stated that between 28 February and March 11, more than 1,100 children were killed or injured as a result of the conflict in Iran, Lebanon, Israel, and several other countries in the region. The report says that among them, 200 children were killed in Iran, 91 in Lebanon, four in Israel, and one in Kuwait.

UNICEF also said that disruptions to education have left millions of children across the region out of school, while some educational infrastructure has been damaged or destroyed.

In a separate report by Save the Children on March 11, it was stated that more than 4 million people have been displaced across several countries in the region as a result of the conflict. The organization also reported that in some areas, children are among the most vulnerable groups. It also noted that in the early hours of the conflict on February 28, an attack on the “Minab” primary school in Iran was reported. According to reports, more than 160 children were killed in the attack. Iranian authorities have blamed the United States for the attack. This incident is described as one of the deadliest child casualty events in the Iran–US and Israel conflict.

Health Situation and Medical Services

According to reports by the Iran Human Rights Monitor (HRA) and Reuters, at least 3,461 people have been killed in Iran so far, including 1,551 civilians and 236 children.

The International Federation of Red Cross and Red Crescent Societies said that at least 1,900 people have been killed and around 20,000 injured in attacks attributed to the United States and Israel. It also referred to 104 additional deaths in an attack on the Iranian naval vessel Dena on March 4.

In Lebanon, 1,189 people have been killed in Israeli attacks so far, including 124 children. In addition, around 400 Hezbollah members have been killed since the conflict began on February 28.

In Iraq, at least 100 people have been killed since the start of the crisis, including civilians and members of affiliated armed groups.

According to the Israeli Ministry of Health, the number of injured in Israel since the beginning of the conflict with Iran reached 6,008 by March 30.

Humanitarian organizations have warned that the continuation of the conflict is placing severe pressure on healthcare services in the region, with hospitals in some areas facing shortages of supplies and a growing number of casualties, reducing their ability to provide emergency care.

Public Concerns: Displacement and Internet Blackouts

A report by the UN High Commissioner for Refugees (UNHCR) published on March 11 states that between 600,000 and 1 million families inside Iran have been temporarily displaced, equivalent to around 3.2 million people.

The report adds that many of these people are moving from major cities, including Tehran, toward northern regions and rural areas. Refugees and migrants, particularly immigrant families from Afghanistan, are among the most vulnerable groups. Many of these migrants, who previously relied on daily wage labor, are now facing severe economic hardship.

Some residents of Tehran told international media, including France 24, about life during the conflict. One man in Tehran said, “I tell my children the sound of explosions is just thunder.” Another woman said, “People are trying to continue their normal lives,” adding that like the past wars, life must go on.

Meanwhile, concerns about internet shutdowns and difficulties in communicating with family members inside and outside the country have increased. According to real-time reports from NetBlocks, internet services in Iran have faced widespread outages since February 28, continuing until March 30, 2026. The disruption has now entered its 31st day and is described as one of the longest internet shutdowns in Iran. This situation has severely affected communication between migrants in Iran and their families, as well as parts of economic activity.

The war between Iran, the United States, and Israel began in a context where the Middle East was already facing a series of political, economic, and security crises. Long-standing tensions between the parties involved had already contributed to regional instability. With the start of the attacks, the conflict has expanded beyond Iran and Israel, with daily reports of damage to civilian infrastructure, public services, and large-scale population displacement.
